When iterating over the ssh config file, iterate over the array linearly
instead of always processing the first and then removing it from the
list. This speeds up processing significantly.

INTERACTIVE_COMMENTS not set, comments were not parsed.
Note there is a remaining edge case where the sourced
file is in fact entered at the comment line.
