Add markup around a few filenames and literal text.

Fix the markup choice in a few instances.

Cuddle up the ending </programlisting> with the previous line.

Turn $FreeBSD$ in to $FreeBSD&dollar; to prevent expansion.

Add Emacs local variables section.
