1733cd34a9 Move the Code of Conduct reporting into its own
separate file.  Link to it from the main Code of
Conduct and vice versa.

The Code of Conduct itself was not changed.

051ced19d3 Add an extra paragraph to separate the law enforcement notification from the
information that should be reported to the committee.  Before, it seemed
confusing so separating these two sentences makes it clear that these are
separate things that do not belong together.

bb615e9522 Announce the new Code of Conduct for the Project.
A lot of work by many people went into making this new code better than
the one we had, including: Valerie Aurora, Anne Dickison, Warner Losh,
Colin Percival, Benno Rice, Devin Teske, and Robert Watson.

b116b2192f Remove mention of 'meritocracy'. This is a term which incorporates
several different meanings and it can be unclear which exactly we
intend.  The paragraph is clear enough without it.

319e698e06 Introduce a new Code of Conduct.
Update the Committer's Guide

  - section 21 covers much of the same areas as the new Code of
    Conduct, but the CoC is authoritative.  Update wording where
    there is some disagreement.

  - Add a new section on Privacy and Confidentiality which was felt to
    be too committer specific to go into the general Code of Conduct.
