98ecd8f450 Clean up Links section: 
Remove section about <anchor>, xml:id makes those unnecessary.
Remove section about using <link> to link to the same document.
Change ulink reference to link.
Add an example showing using an XML empty tag for a link.
Add an example of using <uri>.
Add "Example" to example titles.

0c12038620 - Do not wrap prompts in literal since DB 5.0 doesn' allow it and this 
section is meant to show the rendering, which may be different when
  wrapped into literal

bd7adfb41a Add a section showing use of <email> tags, both with and without 
"nolink".  Move the sample <procedure> inside the <example>, the
comment that noted procedures could not be inside examples is no
longer correct.

2b7913979b Remove a confusing paragraph whose net contribution is negative. Remove 
some &nbsp; from "Ports Collection", it's unnecessary.  Make one example
section consistent with the actual code.
