24df652a45 Move around the text of the Vinum introduction a bit, in an effort to 
make more meaningful the text of the paragraph:

  <para><emphasis>Vinum</emphasis> is a so-called <emphasis>Volume
    Manager</emphasis>, a virtual disk driver that addresses these
    three problems.  Let us look at them in more detail.  Various
    solutions to these problems have been proposed and
    implemented.</para>

which seemed very out of place in the previous version of the text.

3f0d1b56f0 Do what I swore would be done: 
o Remove 4.X information, this includes notes and sections.

o Update documentation to reflect 5.X and 6.X.

o In some areas, try to make the new content version agnostic.

o Skip areas stating "5.[0-5] and later" as it's relevant - we
need a better way to handle these.

For several items, I checked the NOTES files, manual pages,
CVS history, etc.

698a6ee299 Update the newfs example for the newer FreeBSD versions [1] and 
list the old backward compatible instruction for FreeBSD
versions prior to 5.0.

Also the -v option does not work with versions starting
from 5.0.

8af463c4d2 - Various markup fixes: for example, <programlisting></programlisting> 
should be used instead of <para><literal></literal></para>, and
  <screen></screen> instead of <para><command></command></para>
- <screen> tags are not inline elements, so they should not be used
  inside of <para></para>
- s/4.x/4.X and some punctuation fixes
- Some device names fixes I missed in my previous commit

9565589609 Devices name/node tagging is not consistent in the Handbook, it should 
respect the following scheme:
- if the device name is /dev/foo --> <filename>/dev/foo</filename>
- if the device name is just foo --> <devicename>foo</devicename>

f8f3aa5727 Modify indexterms to be more consistent with other disk-related index
