2a6d5f6c26 Add handlers for <chapterinfo>. 
Also add a handler for <contrib> inside of <author> tags.

This should allow us to more formally define contributors to the
Handbook.

65d647eb30 Add markup around a few filenames and literal text. 
Fix the markup choice in a few instances.

Cuddle up the ending </programlisting> with the previous line.

Turn $FreeBSD$ in to $FreeBSD&dollar; to prevent expansion.

Add Emacs local variables section.

9524fc725a Add JADEFLAGS to the nsgmls command line, so that any special "-i" settings 
get passed down properly.  This fixes "make lint" for the Handbook, now that
the chapters are individually buildable.

7d44b5e448 + xref closes with '/>'. This causes a trailing '>' 
at the end of the reference.  Removed the '/'.

+ Marked up 'log' and 'spool' as filenames
  in the discussion of the 'var' directory.

+ Changed fdisk to show up as command.

+ Marked up "sysinstall" using a man.systinstall.8; entity.

213a66a8d3 Introduce a 'NICE_HEADERS' tuneable knob that creates customized 
chapter headers that you may find more aesthetically pleasing than the
rather spartan chapter headers in Norm's print stylesheets.  This
option only effects print output formats for English language books.

Also move the local-en-label-title-sep customization from share/sgml
to en_blah/share/sgml since the best values for this customization
depend on the locale.

This changes (make FORMATS=ps) :

b05f2d4e5b Add an entity &os.numports;, currently set to "5,400", so that we 
don't have to keep this number updated at N different places
throughout the doc tree.

Update 3 instances of "4,000" in the Handbook to use this new entity.

dde382c2b4 Add a dependancy on ${CSS_SHEET} for the docbook.css target. Without this, 
one must run a `make clean' (or `rm docbook.css') or the stylesheet is not
updated, regardless of whether or not it's been updated since the last
build.

1c9cca9012 can not -> cannot

1853b9bcf6 CD-ROM -> CDROM 
O'Reilly has standardized on 'CD-ROM' as have several other
publishers, however 'CDROM' outnumbers 'CD-ROM' 2 to 1 in our
documentation and it has always appeared this way on FreeBSD CDs from
Walnut Creek/BSDi/WRS.  I don't have a preference either way as long
as it stays consistent.

8f28d1f75d Remove Appendix F from the Handbook build. The two most complete 
sections have been moved off into separate tutorials and "Jordan's
picks" of Pentium-class motherboards will just have to be missed ;)

The Handbook is now "only" 681 pages (without the index turned on).
