e18ed558ca Remove SYMLINKS entry from example book Makefile. This appears to be 
unnecessary and only used in the English Porter's Handbook Makefile

98ecd8f450 Clean up Links section: 
Remove section about <anchor>, xml:id makes those unnecessary.
Remove section about using <link> to link to the same document.
Change ulink reference to link.
Add an example showing using an XML empty tag for a link.
Add an example of using <uri>.
Add "Example" to example titles.

3062276fef Add a skeletally brief overview of the FORMATS variable and actually show how 
to build an output file.  Both of these are things might be of more than
academic value to someone working on the documentation.

4d07437c99 The send-pr(1) has been removed in FreeBSD 10, so remove it from most of the 
documentation (books, articles, websites) unless there is an explicit or
"obvious" reason.

Do not touch old news articles, release notes, or the CGI scripts.

e391092606 Remove obsolete example build instructions. Update book and article
