6065548c38 Forced commit to note that the previous commit also ensures that while 
<mediaobject>s are rendered in HTML without the surrounding <div>, the
rest of the code that handles them is incorporated from Norm's
stylesheets.

8b08807ccb Use the txt.install.userconfig general entity to pull in the contents

8bcf72c814 Define a parameter entity for the new txtfiles.ent file, and then use this 
parameter entity to bring the declarations in txtfiles.ent in to scope.

b7e43f0460 The simplest way to use the .txt files that are generated from the .scr 
files is to name them as general entities, listed in this file.  This is
analagous to the approach used to name chapters as external files in
chapters.ent.

bae6e2b3ef Add the ${LOCAL_IMAGES_TXT} variable as a dependency in the html-text 
target, ensuring that any .txt files that need to be generated for the
document will be.

eece7f02d3 Add in the necessary framework so that in the "FORMATS=txt" case, any 
.scr files in the ${IMAGES} list are automatically converted to their
plain text equivalents.

66e8205da4 Fix a long-standing bug in the way (the lack of) images is handled in the 
'text-only' case.

Do this by explicitly setting preferred-mediaobject-notations to the
empty list.

This has only worked for so long because the source format for images in
all the documents with plain text equivalents for images has been EPS,
which isn't used in the HTML output format.  If PNG images were used
then the <textobject> element wasn't chosen.

d53073372f The O'Reilly and Associates book mentioned is now out of print. The 
book's author, Jerry Peek, now maintains an online version of this book,
so refer people to that instead.  [See the message 'Suggestion: add a
link to your document "An MH Primer"' posted to freebsd-doc on 6th
August 2002]

While I'm here, update a broken link to the MH FAQ.

684a9a2b57 Change the section on "Do strip binaries" to "Don't strip binaries 
manually unless you have to." This reflects the INSTALL_PROGRAM
macro, which uses install -s.

d558fc6c47 ncvs.FreeBSD.org is now the main repository host. 
Describe how to commit on ncvs.FreeBSD.org using remote CVS.

Add a note to NOT use the main repository host as your personal
cvs server, as inspired by peter's mail to developers@ a while back.
Refer to the cvsup distribution method and cvsup-master instead and
give an example of an "fcvs" alias, as suggested by many committers.

Reword or remove various paragraphs about not modifiying the repository
yourself, which is no longer possible, since only repomeisters have
access to ncvs.FreeBSD.org.

Remove the paragraph about CVSUMASK, which no longer applies.
