6b07659b86 Added link to Kirk McKusick's order homepage of old BSD releases from 
the Computer Systems Research group.
It covers all BSD versions from 1BSD to 4.4BSD and 4.4BSD-Lite2 (but not
2.11BSD, unfortunately). As well, the last disk holds the final sources
plus the SCCS files. Details at http://www.mckusick.com/csrg/

bab9c37eab Fix up some uses of <screen> and the use of <emphasis> elements within 
and near it. Most of the time this consisted of replacing the <emphasis>
with <replaceable> or <userinput>. Sometimes <screen> is the wrong element
to use, and will need replacing with something like <programlisting> or
<literallayout>.
