1a2cb783d0 Add a question explaining how to get FreeBSD working on one of those 
newfangled IBM Thinkpads with a broken BIOS.  A joint effort between
myself and Bruce (bmah).

While I'm here, change <author> in the header to <corpauthor>, which
is more DocBook'ly correct.

5a65a99ceb Set the heading-level characteristic to '2' if we're generating PDF output. 
This has the useful side effect of generating bookmarks in the PDF file,
which programs like Adobe Acrobat Reader can use to provide a navigable
table of contents in a side window.  This only seems to work with OpenJade,
but the option doesn't cause Jade any problems.

88ac7df539 Move `struct selinfo' and related functions to <sys/selinfo.h>. 
Bump __FreeBSD_version to reflect the move.
For the moment, <sys/select.h> includes <sys/selinfo.h> to allow
clients time to catch up.

Changes made in preparation for SUSv2/POSIX <sys/select.h> requirements.
