- Remove references to CVS, everything is in Subversion now
- The "mentors" file is used in the ports repo, as well as in src/doc
- Commits to "access" should include the new committer's username
- Point users to new source of software. (location name and source have
changed).

We don't have support for multiple <indexterm> elements pointing to
different parts of the same <sect1> right now. It requires a few
radical changes to collateindex.pl, so we can only have _one_ point
for each indexterm in each sect1.

capitalization (which also 'hides' the weird capitalization
from igor, and fixes some of its warnings).
